As a Care Manager you will be providing a community-based service working with adult consumers with medium to long-term serious mental illness, and will work on a Monday to Friday basis.
Our community assessment team provides high quality and comprehensive assessment and treatment for adults and their families living in the Nelson/Tasman region. We enjoy a team culture of collaborative practice, openness, humour and respect across disciplines.

You will be a registered nurse , Occupational Therapist, or Social Worker with the ability to work effectively both within a multidisciplinary team and autonomously. You will engender confidence, trust and rapport with clients and their support networks.
You will be experienced in providing community-centred mental health interventions and ongoing assessments for clients. You will have an up to date knowledge of mental health and associated issues. Experience and knowledge of addictions is desirable. Whilst we are looking for experienced practitioners we recruit primarily for attitude.
