0.4 FTE (16 hours per week)
Based: Hibiscus flexible remote working
Harbour Hospice is the only specialist, palliative care provider for families living in the Hibiscus Coast, North Shore and Warkworth / Wellsford communities. We provide compassionate, free care working primarily with patients, families and carers in their homes, and also within our three hospice sites.
About the role:
This role is part of a multi-disciplinary team that form the Poi (Palliative Outcomes Initiative) Team at Harbour Hospice. It’s a role with heaps of variety and contact with a wide-ranging audience across Aged Residential Care Facilities and GP Practices across the North Shore, Hibiscus Coast and the Warkworth/Wellsford region. The purpose of the role is to provide education, coaching and support to primary and aged residential care providers to build up confidence and knowledge around the delivery of great Palliative Care from a psychosocial perspective.
As a fully NZ qualified Social Worker you will have previous experience working within Palliative Care and/or Grief & Loss and have a good understanding of the bereavement process. You’ll be well connected within the community with the knowledge of knowing what resources are out there to refer providers to. You will be empathetic, an active listener and have great communication skills with the ability to adapt these to a wide-ranging audience. You’ll be a team player who is happy to pitch in and help where needed and have great time management skills with a flexible attitude. Presentation skills particularly those around group work and cultural support would be a great advantage.
Please note: this is not a patient facing role, this role supports the providers who are patient facing and assists them in building up their confidence & education around the psychosocial side of Palliative Care.
